Foot orthotics are specially selected or custom-made insoles designed to support the feet, improve comfort and help influence how forces travel through the body when you stand, walk or run.
At Gav Noble Physiotherapy, we use orthotics as part of a wider clinical approach. That means we do not simply look at your feet in isolation. We assess how your feet, ankles, knees, hips and lower back work together, because a small change at the foot can sometimes make a big difference further up the chain.

Foot orthotics may be suitable for people with:
- Heel pain or plantar fasciitis
- Achilles tendon pain
- Flat feet or over-pronation
- High arches
- Foot fatigue when walking or standing
- Knee, hip or lower back pain linked to foot mechanics
- Sports or running-related overload
- Pressure points or poor shoe support
The Chartered Society of Physiotherapy advises that people with particularly high or low arches may benefit from insoles or orthotics in their shoes, especially when managing heel pain or plantar fascia-related symptoms.
How Do Orthotics Work?
Orthotics do not force the foot into a perfect position. The foot is far too clever, and slightly too stubborn for that.
Instead, orthotics may help by:
- Supporting the arch of the foot
- Improving pressure distribution
- Reducing strain on irritated tissues
- Helping the foot and ankle tolerate activity better
- Improving comfort in everyday footwear
- Supporting walking, running or standing for longer periods
Custom Orthotics And Assessment
Before recommending orthotics, we assess your symptoms, footwear, walking pattern, foot posture, strength, flexibility and activity levels. Where appropriate, we can supply custom-fit orthotics based on your individual needs.
This is important because not everyone needs the same support. A runner with Achilles pain, a golfer with foot fatigue, and someone with heel pain after walking around the shops may all need different advice.
Orthotics can help reduce stress on painful areas, but they are usually most effective when combined with the right exercise plan. For example, plantar fasciitis, Achilles pain and knee pain often need strengthening, mobility work and gradual return to activity.
Research has shown that foot orthoses can be effective in helping pain and function in plantar fasciitis, particularly as part of a wider management plan.
Foot Orthotics Questions
Are custom orthotics better than shop-bought insoles?
Not always. It depends on your symptoms, foot type, activity level and footwear. We will advise what is most appropriate.
Will orthotics weaken my feet?
Not when used properly. They are often used alongside strengthening exercises, not instead of them.
Do I need to wear orthotics forever?
Some people use them long term; others use them while symptoms settle and strength improves.
Can orthotics help knee, hip or back pain?
Sometimes, especially if foot mechanics are contributing to the problem. A proper assessment is needed first.
